The Social Worker serves as a key member of the interdisciplinary care team, supporting Residents and families through assessments, care planning, counseling, advocacy, admissions, discharge planning, behavioral support, and community resource coordination. This role is responsible for evaluating admissions, assessing psychosocial and family needs, coordinating care plans, overseeing behavioral management programs, advocating for Resident rights, and providing support throughout the Resident journey.
