Ensures appropriateness of therapy by: Reviewing patient medication history, assessing patient demographics, recommending formulary alternatives when appropriate, developing drug monitoring plans, interpreting laboratory data, monitoring and documenting pharmacokinetics for targeted drugs, which may include credentialing as a Clinical Pharmacist Practitioner.
