Participates in outreach/meetings in the community, including but not limited to community health fairs, Community Resource Center events, Regional Communities Advisory Committees (RCAC) meetings, CALAIM Providers within their Provider Pod Meetings and other meetings/events as needed. The CHC assists in closing care gaps by providing high quality care to members by advancing health and wellness by delivering proactive, compassion, and equitable whole person care for every member, and CRC visitor.
