This role builds and strengthens referral partnerships, identifies new business opportunities, and supports patient census growth by promoting home health services to healthcare providers and community partners. Strong understanding of home health services, post-acute care, and referral sources, including hospitals, skilled nursing facilities (SNFs), physician offices, and community partners.
