In partnership with clinically trained team members and under clinical guidance, this role utilizes a structured, patient and caregiver centered approach to collect required data for program-specific assessments, review referrals, and determine eligibility based on established criteria, while coordinating access to appropriate clinical, community, and program-based services. Serves as a knowledgeable resource and backup support on available services, including eligibility criteria, access points, and alternative options, and communicates this information clearly to patients, caregivers, and referral sources while actively participating in interdisciplinary team meetings to support coordinated, patient-centered care delivery.
