This Nurse Practitioner role splits time between direct patient care (two days/week) and provider performance work (three days/week), supporting the Medical Director of Provider Performance on clinical chart review, documentation improvement, provider education, and development of educational resources to advance high-quality, value-based care. Assist in developing and delivering provider education, using clinical chart reviews to identify documentation gaps and opportunities to improve clinical reasoning, problem identification, documentation quality, continuity of care, and value-based care practices.
