Essential Job Duties Build and maintain relationships with community stakeholders, Establish and maintain resources and information relating to medical services/programs (e.g., Medicaid) and job opportunities, Actively participate in the Multidisciplinary Treatment Team (MDT), Complete a risk and needs assessment on youth as they enter the Program, and complete re-assessments within established contractual guidelines, Evaluate, develop and document an individual service/care plan (ISP) for the youth based on the assessments, Ensure youth receive applicable assessments within the required timeframe of enrollment and that this data is used in the development of the ISP, Participate in building youth''s ability to function productively in interpersonal relationships, community coping activities and functional skills (e.g.
