Develop care plans in collaboration with patients, peer nav, and primary care physicians Maintain a good working relationship with other personnel, medical staff, and administration Educate and advise on medical issues as needed Follow company policies and procedures: For the proper handling of lab specimens and biohazardous waste Consult with physicians, physician assistants, nurse practitioners and Medical Director as needed Document patient visit/interaction accurately and timely. Utilizing mobile clinics staffed by healthcare professionals, community health workers, and social workers, our teams travel directly to unhoused individuals living in communities across the geographies we serve, providing services where they reside – including tents, cars, shelters, and temporary housing.
