A Day in the Life of this Role: Reporting to the Director of Nursing, the Chronic Pain and Addiction Case Manager provides clinical leadership, care coordination, and nursing support for patients managing chronic pain, opioid-related conditions, and substance use disorders. This role combines complex care management for chronic pain, medication-assisted treatment coordination, patient education, and interdisciplinary collaboration to improve patient safety, treatment adherence, recovery outcomes, and overall quality of life.
