Works with the healthcare team to provide access to community resources and links patients to support services such as social work, nutrition, psychologist, pastoral care, physical therapy and rehabilitation, financial counseling, integrative medicine, pharmacy, legal services, palliative care, hospice, etc. The Nurse Navigator works collaboratively with physicians, advanced practice clinicians, nursing staff, care management, and other members of the healthcare team to promote continuity of care, address barriers, and support a positive patient experience within the Presbyterian Medical Group setting.
