Collaborate closely with physicians, advanced practice providers, clinical staff, patients, and caregivers while accurately documenting assessments, care coordination activities, and provider communications within the EMR. Provide patient education and guidance regarding orthopedic conditions, postoperative recovery, wound care, medications, activity restrictions, and durable medical equipment.
