The APP provides physical and psychological health assessments, formulates diagnosis, orders and interprets diagnostic procedures, prescribes appropriate treatments, manages acute and chronic conditions, and provides patient education and counseling, ensuring continuity of care and respect for individuals in the communities served. Facilitates care planning as appropriate for the patient and care team needs, to include patient/support person education, managing an appropriate length of stay, proper utilization of healthcare resources, follow up planning and discharge orders/prescriptions/forms.
