The Housing Case Manager works closely with their clients in the clients’ homes, in the community, and the office, helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness. Utilizing the vast array of internal services available to clients, look to connect (and coordinate) with other departments including: ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.â¯â¯.
