The Health Services Care Manager, within the framework of the Aging Service Access Points (ASAP) regulations, determines program eligibility and provides a consumer centered approach to assessment, service acquisition, reassessment, and monitoring of the services for the state home care and specialized MassHealth funded programs, to assist elders to live independently in the community. A Bachelors' degree preferred from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least two years of experience in counseling, casework or case management.
