Experience working with underserved populations; knowledge, commitment, and sensitivity to the needs of people with housing insecurity, substance misuse, mental health challenges or other challenging populations A working knowledge of service area social service resources for those patients with behavioral health issues (city, county, state and federal).Ability to work independently. Coordinates and provides supportive services including but not limited to applications, outreach, home visits, social service referrals, crisis intervention, case management, client advocacy and related services to individuals and families assigned to the specific housing program.
