Perform essential counseling responsibilities including screening, intake and orientation, assessment, evaluation and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting and record keeping. To support the mission of Helio Health, the Continuing Care Coordinator will provide rehabilitation services to patients as a part of their overall treatment.
