QUALIFICATIONS: Minimum of a Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or related field, plus two years of experience working with frail older adults. S/he is responsible for conducting in-depth intakes and assessments covering psychological, social and physical, and environmental concerns for frail, Medi-Cal-eligible older adults and their families.
