On the day-to-day, you will: • Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including but not limited to bathing, dressing, grooming, dressing, toileting, and physical lifting when transferring participants • Assist participants with Instrumental ADLs, including but not limited to laundry, light housekeeping, shopping, and light meal preparation • Measure and document participants' vital signs as instructed • Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes • Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed • Provide translation services as needed. Job requirements include: • High school diploma or equivalency • Current Basic Life Support (BLS)/First Aid Certification required • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred • Bilingual preferred • Must be registered with Home Care Registry prior to start date • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.
