Facilitates care by coordinating outside/in-house referrals, and ensuring the necessary information is gathered for complete patient care, including outside medical records, mammograms, and pathology results as the point person to contact. Maintains patient database of all presenting patients including appointments, work up, stage of diagnosis, and survivorship care plans.
