Conducts independent patient visits, including same-day appointments, acute care visits, chronic disease follow-up, preventive care visits, annual wellness visits, transitional care management, and post-hospital discharge follow-up. Functions as a core member of a Team-Based Care pod, collaborating with physicians, registered nurses, medical assistants, and administrative staff to deliver coordinated, patient-centered care.
