The Case Manager provides family-centered care coordination by connecting families with community resources, facilitating access to services, and offering ongoing support, advocacy, and follow-up to help meet their individual needs. 3 - 5 years of experience in maternal-child health, infant and early childhood mental health, parent/family support, care coordination, case management, or related human services fields.
