Your contribution to the team includes: Collaborate with primary care providers and other team members in recognizing and treating mental health conditions, substance use/misuse, subclinical presentations of distress, psychosocial problems, promoting preventive medicine interventions, chronic condition management, and other behavioral needs of our patient population.
