Under the direct supervision of the Director of Care Management and the Lead Social Worker II, The Social Worker I is consulted to provide assessment and referral for services related to the complex care needs of the patient and family for adjustment to illness, crisis intervention and to preserve patient's self determination in the decision making process.
The Social Worker 2 provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. The employee must regularly lift, carry or push/pull less than 10 pounds, frequently lift, carry or push/pull up to 10 pounds, and occasionally lift, carry or push/pull up to 20 pounds.
Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence. Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation.
strong>Minimum QualificationsMust be licensed as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC).
