The Community Services Operations & Field Supervisor is responsible for timely and accurate documentation of assigned activities, including admissions, supervisory visits, participant monitoring, caregiver performance, Service Plans/Plans of Care, concerns, follow-up activities, community engagement activities, and other required records. The employee will work from our Chester office when not performing field responsibilities and will travel throughout Penn Home Health Care's service area for participant admissions, supervisory visits, field support, community engagement and outreach activities, and temporary direct-care coverage when needed.
