Core Responsibilities Include:Provide direct case management and/or coordinate with case managers in the community to provide continuity of service for tenants/clients, including ongoing, active outreachConduct comprehensive tenant assessments and develop action plans to achieve goalsCollaborate with Property Management to coordinate eviction prevention effortsProvide crisis intervention as neededAssist tenants with accessing needed medical, mental health, substance use, and psychosocial supportsAssist tenants in developing basic life skills including maintaining an apartment, and provide budgeting/financial literacy supportLink tenants to employment and skill development opportunities, and build relationships with local community and external agenciesBuild community and peer support among tenants including participation in community programming activitiesAssist tenants as needed with activities of daily living and with transportation needs for medical or mental health-related appointmentsMeet documentation requirements as dictated by program need, including maintenance of clinical records and progress notes Desired Qualifications & ExperienceBachelor's or Master's degree in a social or behavioral science or relevant field or three years of relevant experienceExperience working with people who are homeless, people with mental illness, chronic health issues, and/or addiction disorders, or similar populationsSkilled in crisis response situationsStrong verbal and written communication skillsInterest and experience in working with individuals who are difficult to engage and refer to traditional programs, ability to engage with individuals in their current stage of change. Successful candidates will demonstrate the ability to effectively work with diverse populations in a non-judgmental wayAbility to participate in on-call rotation and occasional evening and weekend work as necessaryValid driver's license and reliable transportation; may be required to transport clients in your personal vehicle Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
