The position provides direct supervision, coaching, and professional support to Family Bridge Community Health Workers and assists in developing effective workflows between Community Health Workers, RN Home Visitors, program support and data staff, healthcare partners, and community organizations. As a Licensed Clinical Social Worker, the position provides social-work expertise to the interdisciplinary team and may provide psychosocial assessment, behavioral-health support, crisis assessment, counseling, care coordination, and clinical social-work consultation consistent with professional licensure, organizational policy, and scope of practice.
