One (1) year of experience working in healthcare, behavioral health, substance use treatment, homeless services, human services, community outreach, or a related setting preferred, including experience working with vulnerable populations with a knowledge of community resources, housing systems, public benefits, behavioral health services, and substance use treatment resources. The CHW uses trauma-informed, person-centered, recovery-oriented, and culturally responsive approaches to engage patients, identify barriers to care, support patient-identified goals, and help patients successfully navigate healthcare and community service systems.
