PATIENT DIAGNOSES: End stage renal disease on dialysis, Chronic kidney disease progression to hemodialysis, Acute kidney injury, Peritoneal dialysis failure/infection requiring change of modality to hemodialysis, Transplant rejection requiring hemodialysis. Call Requirement: NONE; depending on PD ***/hemodialysis night shift census, would be assigned to task within Division departments (not patient assignment).
