The registered dietitian nutritionist (RDN) works within the interprofessional team and with the patient and family providing medical nutrition therapy (MNT); and conducts nutrition assessment, intervention, education, discharge planning, and care coordination to address prevention and treatment of one or more acute or chronic conditions or diseases. Identifies nutrition related problems, determines nutrition diagnosis, prioritizes nutrition intervention and documents plan of care including writing diet orders and educating patient and families on nutrition plan of care.
