Identify self as a contract worker, coordinate patient referrals to a variety of health care services to include, but not limited to, community health nurse, health promotion center, social work service, dietician, clinical nurse specialist, chaplain, and other ancillary services as directed by attending physician. This role will support high-quality patient care across multispecialty clinical settings by assessing patients, administering medications and treatments, coordinating referrals, assisting with procedures, and documenting care accurately in the electronic health record.
