SUMMARY: Systemwide clinical care for patients referred for palliative care consultation and coordination of care across clinical sites, including hospitals, outpatient clinic, and skilled nursing facilities; works with the interdisciplinary palliative care consult team of physicians, advanced practice providers, medical learners, chaplains, and social workers to deliver patient and family care across the Alameda Health System care continuum. Required Experience : One (1) year of experience working in palliative care, hospice or with the serious ill population; five (5) years of nursing experience working in a hospital, clinic, skilled nursing facility and/or home care in a related clinical area (oncology, geriatrics, ICU, neuro).
