OVERVIEW OF POSITION: Plans and conducts daily activities at theparticipant’s home to facilitate individual learning experiences which assistin the development of the participants’ living skills, to build independencyand integration with the community; consistent with the objectives identifiedin the Individual Service Plan (ISP)/Individual Program Plan (IPP), and withthe program design. Helps participants in building independence, to go out and integrate in the community, and carry out activities such as: do the chores of that day, prepare meals, do grocery shopping, utilize public transportation, attend doctor’s appointments, and learn financial management.
