Care coordination and member support: Assist members and caregivers with behavioral health and medical navigation, resource connections, treatment engagement, self-management education, safety planning, discharge follow-up, and continuity of care while maintaining appropriate case management scope. Primary Responsibilities: Staff supervision and daily operations: Provide day-to-day leadership for case managers and care coordination staff, including work assignments, coverage planning, workload monitoring, schedule support, and timely resolution of operational concerns.
