Trauma Level: Facility Specific Job Notes: **Required Experience and Qualifications:** - 2 years of previous experience - State license - BLS, ACLS, and NIHSS certifications - Driver's License - Work history/resume - Skills checklist (completed within 1 year of submission date) - License verification - 1 supervisory reference at the time of submission - Must have a taxable address outside of 50 miles from the facility, unless in Madison with a 10% rate reduction - Not an employee of UnityPoint in the last 12 months **Preferred Experience:** - At least 1 travel contract completed.
