Performs integrated IDT functions encompassing the Occupational Therapist role and the Home Care Coordinator role, ensuring cohesive planning, communication, and oversight of both rehabilitative and home-based support services for PACE participants. Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching of participant, caregivers or other appropriate representatives/family to maintain participant support in the community.
