Minimum Qualifications To serve high-acuity enrollees, applicants must meet one of the following: Bachelor’s degree in a related field with 2 years of relevant experience Licensed Registered Nurse (RN) with 2 years of relevant experience Master’s degree in a related field with 1 year of relevant experience Skills & Competencies Strong interpersonal and communication skills, including the ability to engage with diverse populations Solid knowledge of medical, behavioral health, and community support systems Excellent time management, problem-solving, and multitasking abilities Ability to work independently and within an interdisciplinary team Experience with electronic health records and Microsoft Office (Word, Excel, Outlook) Must be comfortable providing services in community-based settings Typing proficiency of 30+ WPM What We’re Looking For Passionate about improving outcomes for children and families A strength-based, trauma-informed, and culturally responsive approach to care A team player with flexibility, initiative, and a collaborative mindset This role is essential in helping families navigate medical, behavioral, and social service systems through comprehensive care planning, consistent follow-up, and a collaborative team approach.
