Working closely with drivers, the interdisciplinary team (IDT), clinical staff, and the front desk, this role keeps daily routes running smoothly, responds to same-day changes, and ensures transportation is delivered with the patience, dignity, and cultural sensitivity our participants deserve. The Transportation Coordinator plans, organizes, and coordinates the daily transportation of Seen Health PACE participants — to and from the day center and to medically necessary appointments — and manages real-time dispatch so that every participant is picked up and returned safely and on time.
