Key Responsibilities: • Oversee and supervise the daily operation of the HCH Team • Responsible for coordination and tracking of HCH enrollees (new enrollees, discharges, transfers) • Supervise Nurse Care Managers and assign NCM caseloads in partnership with the Assistant Director • Coordinate and/or facilitate health education groups and/or activities for clients, staff, and community • Review, complete, and submit the monthly Attestation report in CyberAccess in a timely manner • Serve as a Cyber Access Practice Administrator maintaining user accounts and monitoring agency usage and prescriber reports • Coordinate management of HIT tools and initiatives, including CyberAccess usage, CMT reports, and hospital admission/discharges • Oversee and/or facilitate staff training on Healthcare Home • Participate in monthly TA calls for HCH Directors and attend state-required meetings and trainings • Oversee Quality Assurance and Quality Improvement for Healthcare Home • Partner effectively with collaborating service areas to meet client needs. Education, Experience, and/or Credential Qualifications: • Current Registered Nurse (RN) in the state of practice • Supervisory experience • Additional Qualifications: • Successful completion of the Criminal Record and Child Abuse & Neglect Background check prior to beginning employment • Completion of New Hire Orientation within 30 Days of employment • All training requirements including Relias Learning within two (2) weeks of employment and annually thereafter • Strong verbal and written communication skills, computer, and organizational skills • Physical Requirements: • The physical requirements and working conditions described here are representative of those an employee encounters while performing the essential functions of this job.
