Duties: Both utilization review and discharge planning - interdisciplinary rounding, family meetings, insurance authorizations and concurrent reviews, advance directives, DME, home health and outpatient referrals, transportation and post-discharge resources; develop a plan of care for each assigned patient from admission to discharge and manage throughput and length of stay. Note: LTAC patients are complex - vents, airways, IV antibiotics, respiratory and wounds; rehab is mostly neuro, amputation, cardiac, trauma and complex medical/surgical.
