Performs thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills and ADL and IADL performance. Attends and contributes to in-services, case conferences, and team meetings as required by the agency to ensure coordinated and comprehensive plans of care for the patients.
