b>Primary Care Nurse Practitioner Requirements
- $45–$55 Per Hour
PSC Community Services, Licensed Home Care Services Agency (LHCSA) is seeking for qualified Register Nurses (RNs) to conduct field visits to patient home and perform assessments in Brooklyn, Queens and Manhattan, as well as some administrative tasks for nursing department (if it's a part-time or full-time employment). - Perform initial assessments on new patients to determine eligibility and needed services.
Act as a primary point of contact for clients, families, and support coordinators, addressing concerns promptly. Oversee and manage the daily operations of the home care office to ensure smooth workflow and service delivery.
