This role supports referral management, monitors hospital and health care portals, maintains account-specific ownership, and helps ensure timely communication from referral through the start of services. Build and maintain professional, service-focused relationships with referral sources, hospital and facility contacts, physicians, discharge planners, case managers, patients, families, and other health care professionals.
