Specific Duties: Perform comprehensive patient evaluations and develop individualized plans of care Provide skilled physical therapy services in patients' homes Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits Monitor patient progress and adjust treatment plans as needed Educate patients and caregivers on exercises, safety, and ongoing care Maintain timely and accurate clinical documentation in compliance with regulations Collaborate with physicians, nurses, and interdisciplinary team members Ensure compliance with Medicare guidelines and home health regulations Qualifications: Degree in Physical Therapy (PT) Active Physical Therapist license (or eligibility) in the state of Connecticut Prior home health experience preferred Strong understanding of patient care, treatment planning, and documentation Excellent communication and organizational skills Ability to work independently and manage a flexible schedule Must have qualifications/experience: NOTE: for LHC GL coding guidance, use the LHC Group Contract Labour GL Coding Guide, available on the LHC Group Home Page.
