You will work closely with the RN Case Manager, Medical Director, Chaplain, Hospice Aide, Administrator, and other members of the Interdisciplinary Group (IDG) to ensure that every patient’s Plan of Care reflects their goals, family dynamics, psychosocial needs, safety concerns, and end-of-life preferences. In this meaningful role, you will provide counseling, education, resource coordination, crisis intervention, and emotional support to patients and families navigating terminal illness and end-of-life care.
