Develops, implements, and updates plans of care; promotes resident-centered care and supports residents in achieving and maintaining their highest practicable level of health and well-being; administers and monitors medications, performs medical treatments and vital sign checks, and verifies physician orders for accuracy and appropriateness. Observes, documents, and reports resident status, including changes in condition and 24-hour reports; promptly communicates concerns to the registered/charge nurse, family, or physician as appropriate; provides thorough transfer of information to the next team leader; follows universal precautions and PPE requirements; and performs other duties as assigned.
