This position works in outpatient clinical settings with frequent interaction among patients, medical providers, behavioral health clinicians, and interdisciplinary teams. Coordinate referrals and collaborate with physicians, nurses, behavioral health providers, pharmacists, care coordinators, Community Health Workers, and other members of the care team.
