Candidate must possess an Illinois nursing license or be willing to obtain one within six (6) months of hire Care Management experience preferred BLS (CPR) required or obtained within 90 days of hire Certification in Case Management preferred Experience working in OB/GYN, Pediatrics, or Care Management of maternal-child populations preferred Strong verbal and written communication and facilitation skills required Strong assessment skills, time management skills, critical thinking skills required Advanced computer skills required Must have ability to interact effectively with a variety of people and situations at all levels of the organization Possess a valid drivers license and reliable transportation Candidates should possess a positive attitude, have a high level of energy, work in a team environment, and be resourceful for an evolving program. The Care Manager will facilitate care through a collaborative process to meet an individual/families comprehensive health needs through various assessments, care coordination, facilitation, and advocacy for options and services to promote quality care, patient safety, and cost-effective outcomes.
