*Evaluate patients/residents to collect data, observe condition and report changes in condition.
Primary Care services include on-site Behavioral Health providers, Social Workers, and Care Management, along with preventive programs like diabetes management and education. Rutland Express Care, located within the same facility as Community Health Rutland, is open seven days a week for the treatment of minor illnesses and injuries for adults and children.
