Reviews, interprets, clarifies, enters, and verifies patient's information and medication orders for processing.\n Ensure that the patient chart is complete for allergies, and concurrent medications (including over the counter or home remedy medications) and documentation of communication with providers/nurses/ patients\n Provide patient counseling, education, and monitoring\n Ensure safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patient\n Assess responses to therapy, patient compliance, and ongoing review of drug regimens; report and manage adverse events.\n The health center provides an array of health care services including but not limited to primary care (adult & pediatric), HIV treatment, HIV Prevention treatment (PrEP/nPEP), and hormone replacement therapies.\n \nPrimary Tasks/Responsibilities:\n\n Ensure the accurate delivery of pharmaceutical care\n Receive physician prescription orders and evaluate the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stability.\n
