Function/Duties of Position: Clinical Activities (not related to teaching or research): Assure the safe, efficient, high quality and customer oriented management of patient care to identified patient populations, including but not limited to completing a comprehensive assessment of the patient’s status; developing, monitoring and revising, as appropriate, an evidence-based treatment plan that addresses diagnoses, patterns of disease, risk factors, and treatment alternatives; treat many common acute and chronic health problems; interpret diagnostic tests prescribe and manage medications and other therapies, provide health teaching with an emphasis on prevention of illness and health maintenance; and, refer patients to other healthcare professionals as necessary. Additionally, the APP is responsible to partner with the patient, family, nursing, pharmacist, and surgical and procedural teams to impact timely and effective discharges and patient flow; support for emergency response and care escalation; impact patient and family experience through communication and education; promote an interdisciplinary approach to care; and support new processes to improve patient outcomes.
